Creating an Icon for my desktop in CS4
I want to use this image as an icon for my desktop

I used the color range tool on the white background to isolate the lips onto a new file in photoshop with a transparent background (checkerboard). But the image still shows up on a white background when I use it as the Icon

HOW DO I GET RID OF THE WHITE?

As Colin said, make your image transparent and Save As .png file.
I then used Pic2Icon application and simply dragged the .png file into the well.
Volia, you now have a transparent icon.

Here is another way. (I work on a Mac or should I say Play on a Mac)
I opened the png file in Preview.
Copied.
Then selected a folder or a file.
File>Get Info.
Select the small icon at the top.
Paste.
I now have star icons with transparent background.
