A rip is they "grey area". Obviously, copyrighted works are copyrighted works. No real DUH about that. The grey area is recreations of other people's labours without acknowledging the source. These are not covered specifically by copyright law, but are frowned upon by professional artists. Especially when it's a commercial venture.
There is designing by yourself, which is amazing
There is designing with inspiration, which is acceptable
There is designing an exact or almost exact duplicate, which is a "rip"
There is taking someone else's work, which is illegal
Note that in the case of tutorials, the finished product has been cleared for public use without acknowledgement (make your own "X"), but copying and posting the steps without acknowledging the author would be in breach of copyright; and listing similar steps without exactly the same wording would be considered the "rip".

No, mostly ripping happens to professional artists. Sometimes it happens though. I am reminded of the Nike/Minor Threat rip where Nike had blatantly stolen work from the punk band Minor Threat to promote a skateboarding festival.
