i need some help in creating a workbook?

i need to design a workbook, the kind where you follow along a teaching on a tape, and fill in answers to questions and take down notes.

the problem i am having is... finding the right format to put it in.

does anyone know of a good program to lay this out in? it can probably be maded in publishing program, but i am unsure on how to start it; a template of some sort would help a lot.

I would use Indesign.
Create a template and stylesheets
Then just flow the text in and format it using the style sheets

Do you have an email address I have a format that you can take into photoshop and customize but it may work like it is. I have it in a .pdf format and it is mine so you don't need to worry about copyright issues. U2U me an email address where I can send it. and its yours.
EDIT: I built it in InDesign so if you want it that way I can do that too it would just be a larger file. You could take the .pdf and rebuild it in InDesign or Quark whatever you want to do it would just get your brain going.
